Lincoln County Health and Human Services Client Rights and Responsibilities
You have the right to …
- Receive a written copy of your rights prior to or at the time of admission.
- To considerate and respectful care.
- To be an active participant in planning treatment, and to ask for more information and have any questions answered fully.
- To an individual treatment plan, and to request an in-house review of that treatment plan.
- To obtain complete, current information concerning your diagnosis, treatment planning, and prognosis, in terms you can understand.
- As a client, you have the right to refuse or terminate treatment at any time with no penalty (unless ordered by court). If you refuse or terminate treatment, the possible risks will be explained to you to allow you to make an informed decision.
- To confidentiality of all communication and records in accordance with state and federal law.
- To an explanation of those rules or regulations which apply to your conduct as a client.
- To reasonable uninterrupted care.
- To obtain information as to any relationship of this facility to other health care and educational institutions insofar as our care is concerned, and to obtain information as to the existence of any professional relationships among individuals, by name, who are treating you.
- To be informed of any significant alternatives for care and treatment that exists.
- To a staff review of, and response to, your opinions, recommendations, and grievances.
- Access to treatment shall be free of discrimination by race, religion, sex ethnicity, age, handicap, or inability to pay.
Client Responsibilities
As the client, you have the responsibility to …
- Be as accurate and complete as possible when asked for information about mental health or medical problems, and drinking/drug use history.
- Actively participate in decisions regarding your treatment and advise staff if you cannot or will not follow a certain treatment plan.
- Be actively involved in following your mutually agreed upon, recommended treatment plan.
- Accept the consequences of your actions if you refuse to follow your treatment plan or accept treatment.
- Advise the treatment staff if you notice any changes in your physical or emotional condition or if you have any concerns or questions about your care.
- Ask for clarification if you do not understand what you have been asked to do, and the reason for such a request.
- Have due consideration for other clients and staff, including their rights to privacy and confidentiality.
- Keep scheduled appointments and when you are unable to do so, for any reason, notify the treatment staff.
- Follow facility rules and regulations affecting client care and conduct.
